Mechanism of Action

Chloramphenicol is a broad-spectrum bacteriostatic antibiotic that inhibits bacterial protein synthesis at the ribosomal level. It specifically and reversibly binds to the 50S ribosomal subunit of susceptible bacteria, interacting with the A2451 and A2452 residues in the 23S rRNA. This binding prevents the peptidyl transferase enzyme from catalysing peptide bond formation, effectively halting the elongation of the growing polypeptide chain.

Unlike macrolide antibiotics that physically block the peptide exit tunnel, chloramphenicol directly interferes with substrate binding at the peptidyl transferase centre. This mechanism is highly effective against a wide range of Gram-positive and Gram-negative bacteria, as well as rickettsiae, chlamydiae, and mycoplasmas. At higher concentrations, chloramphenicol can exert bactericidal activity against certain susceptible organisms such as Haemophilus influenzae.

The selectivity of chloramphenicol for bacterial ribosomes is not absolute. At therapeutic doses, it can also inhibit mitochondrial protein synthesis in mammalian cells, which is the basis for its dose-dependent bone marrow suppression. This effect is particularly pronounced in cells with high metabolic activity, such as bone marrow precursors. The drug is lipid-soluble and penetrates well into tissues and body fluids, including the cerebrospinal fluid, aqueous humour, and pleural cavities, making it valuable for treating infections in otherwise difficult-to-reach anatomical sites.

  • Anaerobic infections: Active against Bacteroides fragilis and other anaerobes, useful in intra-abdominal infections and pelvic inflammatory disease.
