Mechanism of Action

Tacrolimus is a macrolide immunosuppressant that exerts its pharmacological action through specific binding to the immunophilin FK-binding protein 12, forming a complex that inhibits the calcium-dependent serine-threonine phosphatase activity of calcineurin. Under normal physiological conditions, T-cell receptor activation triggers an increase in intracellular calcium, which binds to calmodulin and activates calcineurin. Activated calcineurin dephosphorylates the nuclear factor of activated T-cells, allowing its translocation to the nucleus where it initiates the transcription of cytokine genes, particularly interleukin-2. The tacrolimus-FKBP12 complex binds directly to calcineurin at the interface between its catalytic A subunit and regulatory B subunit, sterically blocking access of NFAT to the calcineurin active site. This prevents NFAT dephosphorylation and nuclear translocation, thereby inhibiting the transcription of IL-2, IL-3, IL-4, IL-5, interferon-gamma, tumour necrosis factor-alpha, and granulocyte-macrophage colony-stimulating factor. Tacrolimus is approximately 10 to 100 times more potent than cyclosporine in vitro and in vivo. The drug also inhibits the release of preformed mediators from mast cells and basophils, downregulates IL-2 receptor expression on activated T-lymphocytes, and suppresses both humoral and cell-mediated immune responses with particular efficacy in preventing allograft rejection in solid organ transplantation.
