Mechanism of Action

Liposomal amphotericin B is a broad-spectrum polyene antifungal agent that works by binding to ergosterol, the principal sterol component of fungal cell membranes. The drug is encapsulated within unilamellar liposomes composed of hydrogenated soy phosphatidylcholine, distearoylphosphatidylglycerol, and cholesterol. This liposomal formulation is designed to carry amphotericin B safely through the bloodstream while minimizing interaction with mammalian cell membranes. When the liposome encounters a fungal cell, it binds to the cell wall and transits intact through the cell wall matrix — a process made possible by the viscoelastic and deformable properties of the fungal cell wall. Once the liposome reaches the fungal cell membrane, amphotericin B is released because it has a ten-fold higher binding affinity for ergosterol compared to cholesterol in the liposome. The released amphotericin B molecules insert into the fungal cell membrane and assemble into transmembrane pores or ion channels. These pores disrupt the membrane's selective permeability, causing leakage of essential intracellular ions such as potassium, magnesium, and calcium, along with other small molecules. The resulting ionic imbalance and metabolic disruption lead to rapid fungal cell death. Additionally, amphotericin B induces oxidative damage within the fungal cell through generation of reactive oxygen species. The liposomal formulation significantly reduces the nephrotoxicity and infusion-related reactions associated with conventional amphotericin B deoxycholate.
