Mechanism of Action

Aztreonam is a synthetic bactericidal antibiotic that represents the first member of the monobactam class of beta-lactams. It works by selectively binding to penicillin-binding proteins (PBPs), with a high and specific affinity for PBP-3 of susceptible aerobic Gram-negative bacteria. Penicillin-binding proteins are essential enzymes involved in the final transpeptidation step of bacterial cell wall peptidoglycan synthesis. By inhibiting PBP-3, aztreonam prevents the cross-linking of peptidoglycan chains, disrupting the structural integrity of the bacterial cell wall and resulting in cell elongation, lysis, and cell death. Due to its unique monocyclic beta-lactam ring structure, aztreonam is highly resistant to hydrolysis by many beta-lactamase enzymes, including metallo-beta-lactamases and cephalosporinases, making it effective against multi-drug resistant Gram-negative pathogens. However, it lacks activity against Gram-positive bacteria and anaerobes because it does not bind to their respective PBPs. Unlike other bicyclic beta-lactams, such as penicillins and cephalosporins, aztreonam exhibits minimal cross-reactivity with their respective antibodies, allowing it to be used safely as an alternative therapy in patients with a history of severe penicillin allergies.
