Mechanism of Action

Bortezomib is a first-in-class proteasome inhibitor that works by reversibly blocking the 26S proteasome, a large protein complex inside cells that acts as the cell's quality control system for breaking down unwanted or damaged proteins. Normal cells rely on the proteasome to maintain a delicate balance of proteins that regulate cell growth, division, and death. Cancer cells, particularly multiple myeloma cells, produce large quantities of abnormal proteins and are especially dependent on the proteasome to survive. When bortezomib inhibits the proteasome, these abnormal proteins accumulate to toxic levels inside the cancer cell. This accumulation triggers several death pathways: it prevents the activation of nuclear factor kappa-B, a protein that normally helps cancer cells survive; it causes the buildup of pro-apoptotic proteins that push the cell toward programmed cell death; and it induces endoplasmic reticulum stress as the cell becomes overwhelmed with misfolded proteins. Bortezomib is the first proteasome inhibitor approved for clinical use and has transformed the treatment of multiple myeloma. It is given by injection either intravenously or subcutaneously and is often combined with lenalidomide and dexamethasone.
