Mechanism of Action
Bortezomib is a first-in-class proteasome inhibitor that works by reversibly blocking the 26S proteasome, a large protein complex inside cells that acts as the cell's quality control system for breaking down unwanted or damaged proteins. Normal cells rely on the proteasome to maintain a delicate balance of proteins that regulate cell growth, division, and death. Cancer cells, particularly multiple myeloma cells, produce large quantities of abnormal proteins and are especially dependent on the proteasome to survive. When bortezomib inhibits the proteasome, these abnormal proteins accumulate to toxic levels inside the cancer cell. This accumulation triggers several death pathways: it prevents the activation of nuclear factor kappa-B, a protein that normally helps cancer cells survive; it causes the buildup of pro-apoptotic proteins that push the cell toward programmed cell death; and it induces endoplasmic reticulum stress as the cell becomes overwhelmed with misfolded proteins. Bortezomib is the first proteasome inhibitor approved for clinical use and has transformed the treatment of multiple myeloma. It is given by injection either intravenously or subcutaneously and is often combined with lenalidomide and dexamethasone.
Therapeutic Uses & Indications
Therapeutic Uses
Bortezomib is prescribed for the treatment of:
- Multiple Myeloma: The primary indication. Bortezomib is used as first-line therapy for newly diagnosed multiple myeloma, either alone or in combination with lenalidomide, dexamethasone, melphalan, and prednisone. It is also indicated for relapsed or refractory disease.
- Mantle Cell Lymphoma: Indicated for the treatment of adult patients with mantle cell lymphoma who have received at least one prior therapy, offering an important option for this aggressive B-cell lymphoma.
Why Manufacturers Choose Bortezomib
Bortezomib is a blockbuster oncology drug that revolutionized multiple myeloma treatment and remains a cornerstone of myeloma therapy worldwide. The expiry of key patents has opened the generic market, and Indian manufacturers with sterile injectable capabilities are well-positioned to capture significant market share. The drug is listed on the WHO Essential Medicines List and is part of standard treatment protocols globally. The Indian domestic market for bortezomib is substantial, driven by the large multiple myeloma patient population and increasing access to cancer care. Export opportunities to regulated and semi-regulated markets are significant for manufacturers with WHO-GMP and USFDA-compliant facilities.
This makes Bortezomib one of the highest-value oncology injectables for third party manufacturing in India, with consistent demand from hospital tender business, PCD franchise operators, and international generic drug buyers.
