Mechanism of Action

Calcium Folinate, also known as Leucovorin Calcium, is a reduced form of folic acid that serves as an essential coenzyme in nucleic acid synthesis. Unlike folic acid, which requires conversion by the enzyme dihydrofolate reductase to become active, calcium folinate is already in its reduced form and can be used directly by cells without this enzymatic step. This property is the basis for its two primary roles in cancer therapy. First, it acts as a rescue agent after high-dose methotrexate therapy. Methotrexate works by blocking dihydrofolate reductase, preventing cells from making active folate. By administering calcium folinate after methotrexate, normal cells receive the active folate they need, effectively rescuing them from methotrexate toxicity while cancer cells remain affected. Second, calcium folinate enhances the activity of 5-fluorouracil by stabilizing the binding of its active metabolite to thymidylate synthase, the enzyme responsible for DNA repair and replication. This makes the blockade more effective, increasing cancer cell death. Calcium folinate is also used to treat megaloblastic anemia caused by folic acid deficiency and to counteract toxicity from other folic acid antagonists.

Therapeutic Uses & Indications

Therapeutic Uses

Calcium Folinate (Leucovorin Calcium) is indicated for:

  • Methotrexate Rescue: Used after high-dose methotrexate therapy in osteosarcoma and other cancers to protect normal bone marrow and gastrointestinal cells from methotrexate toxicity while preserving the anticancer effect on tumor cells.
  • Colorectal Cancer: Used in combination with 5-fluorouracil to prolong survival in the palliative treatment of advanced colorectal cancer by enhancing the cytotoxic effect of 5-FU through thymidylate synthase inhibition.
  • Megaloblastic Anemia: For the treatment of megaloblastic anemias due to folic acid deficiency when oral therapy is not feasible, including in malabsorption syndromes, pregnancy, liver disease, and malnutrition.
  • Folic Acid Antagonist Overdose: As an antidote for overdosage of folic acid antagonists including methotrexate, pyrimethamine, and trimethoprim, counteracting both therapeutic and toxic effects.

Why Manufacturers Choose Calcium Folinate

Calcium Folinate is an essential drug in modern oncology practice, listed on the WHO Essential Medicines List. Its dual role as a methotrexate rescue agent and a 5-FU modulator ensures consistent demand across all cancer care settings. In India, the injectable form at 50mg/vial and 100mg/vial strengths is widely used in hospital oncology departments and infusion centers. Unlike many cytotoxic drugs, calcium folinate does not require specialized cytotoxic handling facilities for manufacturing, making it accessible to a wider range of sterile injectable manufacturers while still commanding consistent prescription volumes.

This makes Calcium Folinate one of the most consistently demanded oncology support drugs for third party manufacturing in India, with year-round demand from hospital tender business and PCD franchise networks.
