Mechanism of Action

Cefixime and Ornidazole work through two distinct and complementary mechanisms to cover both aerobic bacteria and anaerobic or protozoal pathogens in a single formulation. Cefixime, a third-generation cephalosporin, exerts its bactericidal effect by binding to penicillin-binding proteins located in the bacterial cell wall, specifically inhibiting the transpeptidase enzymes that catalyse the cross-linking of peptidoglycan strands. This disruption of cell wall synthesis leads to osmotic instability and bacterial cell death. Cefixime is particularly effective against aerobic Gram-negative organisms such as Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Neisseria gonorrhoeae, as well as Gram-positive organisms like Streptococcus pyogenes and Streptococcus pneumoniae. Ornidazole belongs to the 5-nitroimidazole class and acts through a fundamentally different mechanism. It enters microbial cells by diffusion, where its nitro group is reduced by bacterial nitroreductase enzymes under anaerobic conditions to form toxic intermediate compounds and free radicals. These reactive species damage microbial DNA by causing strand breakage, helix destabilisation, and inhibition of nucleic acid synthesis, ultimately leading to cell death. Ornidazole is active against anaerobic bacteria such as Bacteroides fragilis, Clostridium species, and Fusobacterium, as well as protozoal parasites including Entamoeba histolytica, Giardia intestinalis, and Trichomonas vaginalis.
