Mechanism of Action

Cisplatin is a first-generation platinum-based chemotherapy drug that works by directly damaging DNA in cancer cells. After entering the cell through passive diffusion and active transport via copper transporter CTR1, cisplatin undergoes aquation in the low-chloride intracellular environment. This process replaces its chloride ligands with water molecules, creating a highly reactive positively charged species. This activated cisplatin then binds preferentially to the N7 position of guanine bases in DNA, forming primarily intrastrand cross-links between adjacent guanine residues (1,2-d(GpG) adducts) which account for approximately 90% of all adducts. The remaining adducts include intrastrand ApG cross-links and interstrand cross-links. These covalent cross-links distort the DNA double helix, causing significant bending and unwinding that blocks DNA replication and transcription. The resulting DNA damage activates the ATM/ATR-mediated DNA damage response, leading to G2/M cell cycle arrest. If the damage is irreparable, the cell undergoes apoptosis through both the intrinsic mitochondrial pathway and the extrinsic death receptor pathway.
