Mechanism of Action

Clobazam works by potentiating GABAergic neurotransmission in the central nervous system. It binds to the benzodiazepine site of the GABA-A receptor complex, acting as a positive allosteric modulator that increases the affinity of the receptor for gamma-aminobutyric acid (GABA), the primary inhibitory neurotransmitter in the brain. This binding enhances chloride ion conductance through the ionophore, leading to hyperpolarization of the neuronal membrane and reduced neuronal excitability.

Unlike classical 1,4-benzodiazepines, clobazam is a 1,5-benzodiazepine with a distinct pharmacological profile. It has lower affinity for the alpha-1 subunit of the GABA-A receptor, which is primarily responsible for sedative effects, and higher affinity for the alpha-2 subunit, which mediates anxiolytic and anticonvulsant activity. This selectivity results in effective seizure control with comparatively less sedation. The major active metabolite, N-desmethylclobazam (norclobazam), has a longer half-life than the parent compound and contributes significantly to the sustained anticonvulsant effect. Clobazam is metabolized primarily via CYP2C19 and CYP3A4 enzymes in the liver.
