Mechanism of Action
Clomifene Citrate works as a selective estrogen receptor modulator that induces ovulation by interfering with estrogen feedback mechanisms in the hypothalamic-pituitary-ovarian axis. It blocks estrogen receptors in the hypothalamus, tricking the body into perceiving that circulating estrogen levels are low. This perceived deficiency triggers increased secretion of gonadotropin-releasing hormone from the hypothalamus, which in turn stimulates the anterior pituitary to release higher levels of follicle-stimulating hormone and luteinizing hormone.
The rise in FSH promotes follicular recruitment and maturation in the ovaries, while the subsequent LH surge triggers ovulation. Clomifene exists as a mixture of two stereoisomers: enclomiphene and zuclomiphene. Enclomiphene is the more potent isomer with greater antiestrogenic activity and is primarily responsible for inducing follicular development, while zuclomiphene has a longer half-life and contributes to sustained receptor occupancy. The drug exerts both estrogenic and antiestrogenic effects depending on the target tissue, with its antiestrogenic action at the hypothalamic level being central to its ovulation-inducing properties.
Therapeutic Uses & Indications
Clomifene Citrate is a first-line oral medication for ovulation induction in women with anovulatory or oligo-ovulatory infertility.
- Polycystic Ovary Syndrome: The primary indication, where clomifene helps induce ovulation in women with PCOS-related anovulatory infertility.
- Unexplained Infertility: Used in combination with intrauterine insemination to induce superovulation and increase conception chances.
- Oligo-ovulation: Effective for women with infrequent or irregular ovulation due to hypothalamic-pituitary dysfunction.
- Post-Contraceptive Amenorrhea: Helps restore ovulatory cycles in women who have not resumed ovulation after discontinuing hormonal contraceptives.
- Male Infertility: Used off-label to treat oligospermia by stimulating endogenous gonadotropin production.
Clomifene Citrate is one of the most widely prescribed fertility drugs globally and is listed on the WHO Model List of Essential Medicines. In India, it is manufactured predominantly as 50mg tablets and is a staple product for PCD franchise companies serving the gynaecology and fertility segments. The molecule enjoys consistent demand due to its affordability, oral route of administration, and well-established safety profile over more than five decades of clinical use. Indian manufacturers produce clomifene citrate to IP, BP, and USP standards, supplying both the domestic market and export destinations across Asia, Africa, and the Middle East.
