Mechanism of Action

Clotrimazole is a broad-spectrum imidazole antifungal agent that works primarily by disrupting the structure and function of the fungal cell membrane. It target-blocks the enzyme lanosterol 14-alpha-demethylase, which is a cytochrome P450-dependent enzyme responsible for converting lanosterol into ergosterol. Ergosterol is an essential sterol required to maintain the stability, fluidity, and integrity of the fungal cell membrane. When clotrimazole inhibits this enzyme, ergosterol synthesis is depleted, and precursor methylsterols accumulate within the cell membrane.

This disruption leads to increased membrane permeability, causing the leakage of essential intracellular elements, such as potassium, and eventually cell death. Clotrimazole also inhibits fungal mitochondrial and peroxisomal enzymes, resulting in a toxic buildup of hydrogen peroxide within the cell, which contributes to its fungicidal effect. At lower clinical concentrations, it is fungistatic, while at higher concentrations, it becomes fungicidal, providing effective clearance of superficial fungal infections.
