Mechanism of Action
Colistin Sulphate, also known as Polymyxin E, is a cationic polypeptide antibiotic that exerts its bactericidal activity by disrupting the integrity of the Gram-negative bacterial cell membrane. The mechanism begins with the polycationic cyclic peptide interacting with the negatively charged lipopolysaccharide molecules in the outer membrane of Gram-negative bacteria. This electrostatic interaction displaces calcium and magnesium ions that normally stabilize the lipopolysaccharide layer, leading to disruption of the outer membrane structure. Once the outer membrane is compromised, colistin inserts its hydrophobic domains into the cytoplasmic membrane, causing increased permeability and loss of cellular contents. This results in rapid bacterial cell death through leakage of intracellular components including nucleic acids and ions. Colistin acts primarily against Gram-negative bacteria, including multidrug-resistant strains such as Pseudomonas aeruginosa, Acinetobacter baumannii, and Klebsiella pneumoniae, which are increasingly difficult to treat with conventional antibiotics. Unlike many other antibiotics that target specific bacterial enzymes or protein synthesis pathways, colistin's membrane-disrupting mechanism makes it less prone to the development of resistance. Due to its potent activity against carbapenem-resistant organisms, colistin has been classified as a critically important antimicrobial by the World Health Organization and is reserved as a last-line treatment option for serious infections where other agents have failed.
Therapeutic Uses & Indications
Therapeutic Uses
Colistin Sulphate is indicated for the treatment of infections caused by susceptible Gram-negative bacteria:
- Multidrug-Resistant Gram-Negative Infections: Reserved as a last-line treatment for serious infections caused by carbapenem-resistant Pseudomonas aeruginosa, Acinetobacter baumannii, and Klebsiella pneumoniae, including hospital-acquired pneumonia, ventilator-associated pneumonia, and bloodstream infections.
- Gastrointestinal Infections: Used orally for bowel decontamination and treatment of gastrointestinal infections caused by enteropathogenic E. coli and other susceptible Gram-negative organisms.
- Pediatric Oral Suspension: Available as an oral suspension for pediatric use in treating diarrheal diseases and for selective decontamination of the digestive tract in immuno-compromised children.
- Topical Use: Incorporated into topical creams, powders, and otic solutions for superficial Gram-negative infections.
Why Manufacturers Choose Colistin Sulphate
Colistin Sulphate occupies a unique position in the antibiotic manufacturing landscape as a critical last-resort antimicrobial. With the rising global incidence of multidrug-resistant Gram-negative infections, demand for colistin has increased substantially, particularly in hospital settings. Indian manufacturers have established expertise in producing colistin sulphate via fermentation technology and are among the world's leading suppliers of this API. The molecule is available in multiple dosage forms including tablets, oral suspension, and powder for topical use, offering manufacturers diverse formulation options. WHO-GMP certification is especially important for this molecule given its classification as a Reserve-group antibiotic in the WHO AWaRe classification. The growing export demand from regions with high burden of antimicrobial resistance makes Colistin Sulphate a strategic addition to any antibiotic manufacturing portfolio.
