Mechanism of Action

Dexlansoprazole is a proton pump inhibitor that belongs to the substituted benzimidazole class of antisecretory compounds. It is the R-enantiomer of lansoprazole and suppresses gastric acid secretion by specific inhibition of the H+, K+-ATPase enzyme system at the secretory surface of gastric parietal cells. This enzyme complex is responsible for the final step of gastric acid production, where hydrogen ions are exchanged for potassium ions across the parietal cell membrane. Dexlansoprazole accumulates in the acidic environment of the parietal cell canaliculi, where it is protonated and converted to its active sulfenamide form. This active species forms covalent disulfide bonds with cysteine residues on the proton pump, resulting in irreversible inhibition of acid secretion. Because the H+, K+-ATPase is the terminal step in acid production, dexlansoprazole effectively blocks both basal and stimulated gastric acid secretion regardless of the stimulus.
