Mechanism of Action

Disulfiram works by irreversibly inhibiting aldehyde dehydrogenase (ALDH), a key hepatic enzyme involved in the oxidative metabolism of alcohol. Normally, alcohol is first metabolized by alcohol dehydrogenase to acetaldehyde, which is then rapidly converted to acetate by aldehyde dehydrogenase. Disulfiram binds to the active site of ALDH, specifically competing with nicotinamide adenine dinucleotide (NAD) at the cysteine residue, rendering the enzyme inactive. When a person taking disulfiram consumes alcohol, acetaldehyde accumulates in the blood to levels five to ten times higher than normal. This buildup triggers a highly unpleasant systemic reaction known as the disulfiram-alcohol reaction, characterized by facial flushing, throbbing headache, nausea, vomiting, chest pain, palpitations, hyperventilation, tachycardia, hypotension, and vertigo. The reaction begins within 5 to 15 minutes of alcohol ingestion and can last from 30 minutes to several hours. The severity is proportional to both the dose of disulfiram and the amount of alcohol consumed. Disulfiram itself does not reduce craving for alcohol or treat withdrawal symptoms. Instead, it acts as a psychological deterrent, where the knowledge that drinking will cause immediate and severe discomfort motivates patients to maintain abstinence. This makes disulfiram an effective pharmacological adjunct in comprehensive alcohol dependence treatment programs.

Therapeutic Uses & Indications

Therapeutic Uses

Disulfiram is prescribed for the treatment of:

  • Alcohol Dependence: As an aversive therapy for selected chronic alcohol patients who are motivated to remain abstinent. It is used in conjunction with supportive psychotherapy, counseling, and behavioral interventions as part of a comprehensive alcohol use disorder management program.
  • Off-Label and Investigational Uses: Emerging research suggests potential applications in cocaine dependence, certain malignancies (as a proteasome inhibitor and DNA demethylating agent), and drug-resistant fungal infections, though these are not yet approved indications in most countries.

Why Manufacturers Choose Disulfiram

Disulfiram has been a mainstay in alcohol dependence pharmacotherapy for over six decades, with a well-established safety profile and predictable pharmacology. Although it is not a high-volume molecule compared to antibiotics or cardiovascular drugs, it occupies a unique niche in the neurological and psychiatric therapeutic segment with stable prescription demand. Indian manufacturers produce disulfiram 250mg tablets for both the domestic market and export to regulated markets, particularly the US and Europe, where alcohol use disorder affects a significant population. The manufacturing process is straightforward, and the tablet formulation is standardized across IP, USP, and BP pharmacopoeias. Recent research interest in disulfiram's potential anticancer and antimicrobial properties may open new avenues for market expansion. For PCD franchise operators, disulfiram offers a differentiated product in the neurology segment with steady demand from de-addiction centers and psychiatric practices.

This makes Disulfiram a valuable molecule for third party manufacturing in India, with consistent demand from super-specialty PCD franchise networks.
