Mechanism of Action

Domperidone is a peripherally acting dopamine D2 and D3 receptor antagonist with both antiemetic and prokinetic properties. Unlike metoclopramide, domperidone does not readily cross the blood-brain barrier, which significantly reduces the risk of central nervous system side effects such as extrapyramidal reactions. Its antiemetic effect is mediated through antagonism of dopamine D2 receptors in the chemoreceptor trigger zone (CTZ) located in the area postrema of the medulla oblongata, a region that lies outside the blood-brain barrier and is accessible to circulating domperidone. By blocking dopamine receptors in the CTZ, domperidone suppresses the signals that trigger nausea and vomiting.
