Mechanism of Action

Efavirenz is a non-nucleoside reverse transcriptase inhibitor (NNRTI) that specifically targets the reverse transcriptase enzyme of human immunodeficiency virus type 1 (HIV-1). Unlike nucleoside reverse transcriptase inhibitors that compete at the active site, efavirenz binds directly to a non-catalytic allosteric site on HIV-1 reverse transcriptase, inducing a conformational change that disrupts the enzyme's catalytic activity. This binding inhibits both the RNA-dependent and DNA-dependent DNA polymerase activities of the enzyme, effectively blocking the transcription of viral RNA into DNA — an essential step in the HIV replication cycle. By preventing the generation of DNA copies of viral RNA, efavirenz halts the synthesis of new virions and reduces the viral load in infected individuals. Efavirenz is highly selective for HIV-1 reverse transcriptase and does not inhibit HIV-2 reverse transcriptase or human cellular DNA polymerases alpha, beta, gamma, or delta, contributing to its specificity. As a first-generation NNRTI, efavirenz has a low genetic barrier to resistance, meaning a single mutation (particularly the K103N mutation in the reverse transcriptase gene) can confer high-level resistance.
