Mechanism of Action
Entecavir is a guanosine nucleoside analogue that inhibits hepatitis B virus (HBV) replication through a unique three-step mechanism. After cellular uptake, entecavir undergoes phosphorylation by cellular kinases to form its active triphosphate metabolite, entecavir triphosphate. This active form competes with the natural substrate deoxyguanosine triphosphate for incorporation into viral DNA by the HBV reverse transcriptase. Once incorporated into the growing DNA chain, entecavir triphosphate causes chain termination by preventing the addition of further nucleotides, effectively halting viral DNA synthesis. Unlike some other nucleoside analogues, entecavir inhibits all three stages of HBV replication: priming, reverse transcription of the negative strand, and synthesis of the positive strand of HBV DNA. This comprehensive blockade results in potent suppression of viral load. Entecavir has a high genetic barrier to resistance, meaning the virus finds it difficult to develop mutations that evade the drug, making it a first-line treatment for chronic hepatitis B. It shows minimal mitochondrial toxicity and has negligible effects on cellular DNA polymerases, contributing to its favourable safety profile even during long-term therapy.
Therapeutic Uses & Indications
Therapeutic Uses
Entecavir is prescribed for the treatment of:
- Chronic Hepatitis B Virus Infection: Entecavir is indicated for adults and paediatric patients aged 2 years and older with evidence of active viral replication and active liver inflammation. It suppresses HBV DNA to undetectable levels, normalises liver enzymes, and improves liver histology.
- Compensated and Decompensated Liver Disease: Entecavir is used in patients with chronic HBV infection who have compensated liver function as well as those with decompensated liver disease, helping to prevent disease progression to cirrhosis and hepatocellular carcinoma.
- Prevention of HBV Recurrence After Liver Transplant: It is used prophylactically to prevent hepatitis B reinfection in patients undergoing liver transplantation.
- Lamivudine-Refractory Patients: A higher dose of 1 mg daily is used in patients with known lamivudine resistance or a history of hepatitis B viremia while on lamivudine therapy.
Why Manufacturers Choose Entecavir
Entecavir is one of the most sought-after antiviral molecules for third party manufacturing in India, driven by its inclusion in the WHO Essential Medicines List and consistent global demand. Indian manufacturers benefit from a well-established supply chain for nucleoside analogues and cost-competitive production capabilities. The molecule is available in 0.5 mg and 1 mg tablet strengths, both of which are high-volume SKUs in domestic and export markets. With patent expirations opening generic markets worldwide, entecavir offers strong export opportunities to regulated markets including the US, Europe, and Southeast Asia. Its high barrier to resistance and superior safety profile make it a preferred choice over older antivirals like lamivudine. This makes Entecavir one of the most sought-after molecules for third party manufacturing in India, with consistent demand from PCD franchise operators and export-oriented companies.
