Mechanism of Action

Entecavir is a guanosine nucleoside analogue that inhibits hepatitis B virus (HBV) replication through a unique three-step mechanism. After cellular uptake, entecavir undergoes phosphorylation by cellular kinases to form its active triphosphate metabolite, entecavir triphosphate. This active form competes with the natural substrate deoxyguanosine triphosphate for incorporation into viral DNA by the HBV reverse transcriptase. Once incorporated into the growing DNA chain, entecavir triphosphate causes chain termination by preventing the addition of further nucleotides, effectively halting viral DNA synthesis. Unlike some other nucleoside analogues, entecavir inhibits all three stages of HBV replication: priming, reverse transcription of the negative strand, and synthesis of the positive strand of HBV DNA. This comprehensive blockade results in potent suppression of viral load. Entecavir has a high genetic barrier to resistance, meaning the virus finds it difficult to develop mutations that evade the drug, making it a first-line treatment for chronic hepatitis B.
