Mechanism of Action

Eprosartan Mesylate is a potent, selective angiotensin II receptor blocker (ARB) that exerts its antihypertensive effect by inhibiting the renin-angiotensin-aldosterone system (RAAS). It competitively and selectively blocks the binding of angiotensin II to the angiotensin II type 1 (AT1) receptor, which is found in vascular smooth muscle, adrenal gland, and other tissues. Angiotensin II is a powerful vasoconstrictor that also stimulates aldosterone secretion, promotes sodium and water retention, and activates sympathetic nervous system activity. By occupying the AT1 receptor, eprosartan prevents angiotensin II from exerting these effects, leading to vasodilation of both resistance and capacitance vessels, reduced peripheral vascular resistance, and lower blood pressure. Unlike some other ARBs, eprosartan has a unique non-biphenyl, non-tetrazole structure that gives it a distinct binding profile with rapid association and dissociation kinetics at the AT1 receptor. It also blocks angiotensin II receptors on sympathetic nerve endings, reducing norepinephrine release and further contributing to blood pressure lowering. Eprosartan does not inhibit ACE (kininase II) and therefore does not affect bradykinin metabolism, resulting in a lower incidence of cough compared to ACE inhibitors. The drug has no partial agonist activity at the AT1 receptor and its affinity for the AT1 receptor is approximately 1,000 times greater than for the AT2 receptor.
