Mechanism of Action
Erythromycin is a macrolide antibiotic that inhibits bacterial protein synthesis by binding reversibly to the 50S subunit of the bacterial ribosome. Specifically, it binds to the 23S ribosomal RNA within the 50S subunit at a site near the peptidyl transferase centre. This binding blocks the translocation step of protein elongation by preventing the movement of the peptidyl-tRNA from the A-site (aminoacyl site) to the P-site (peptidyl site) of the ribosome. As a result, the growing peptide chain cannot be extended, and protein synthesis is halted. Erythromycin is primarily bacteriostatic, meaning it stops bacterial growth rather than directly killing bacteria, but it can be bactericidal at higher concentrations against highly susceptible organisms. It has activity against a broad range of gram-positive bacteria, some gram-negative bacteria, and atypical pathogens including Mycoplasma, Chlamydia, and Legionella species. In addition to its antibacterial action, erythromycin is a motilin receptor agonist in the gastrointestinal tract, stimulating gastric motility and acting as a prokinetic agent.
