Mechanism of Action
Ezetimibe works by selectively inhibiting the absorption of cholesterol from the small intestine. It targets the Niemann-Pick C1-Like 1 (NPC1L1) protein, which is located on the brush border membrane of intestinal epithelial cells and hepatocytes. This protein plays a critical role in the transport of dietary and biliary cholesterol across the intestinal wall into the bloodstream. By binding to NPC1L1, ezetimibe prevents cholesterol from being taken up by enterocytes, thereby reducing the delivery of intestinal cholesterol to the liver.
This reduction in hepatic cholesterol stores triggers a compensatory increase in the clearance of cholesterol from the blood through upregulation of LDL receptors on liver cells.
