Mechanism of Action

Famciclovir is a prodrug that is rapidly converted to its active metabolite, penciclovir, after oral administration. Penciclovir is a guanosine nucleoside analogue that inhibits viral DNA replication. Once inside virus-infected cells, penciclovir is phosphorylated by viral thymidine kinase to penciclovir monophosphate, which is then further phosphorylated by cellular enzymes to penciclovir triphosphate, the active form of the drug.

Penciclovir triphosphate competitively inhibits viral DNA polymerase by mimicking the natural substrate deoxyguanosine triphosphate. It incorporates into the growing viral DNA chain, causing chain termination and preventing further viral replication. The selectivity of famciclovir for virus-infected cells is high because the initial phosphorylation step relies on viral thymidine kinase, which is present only in herpesvirus-infected cells. Uninfected cells phosphorylate the drug poorly, minimizing systemic toxicity.

Penciclovir triphosphate has a long intracellular half-life of 7 to 20 hours in herpesvirus-infected cells, depending on the virus type. This prolonged intracellular persistence allows for less frequent dosing compared to other antivirals. Famciclovir is active against herpes simplex virus types 1 and 2, varicella-zoster virus, and Epstein-Barr virus.
