Mechanism of Action

Fexofenadine hydrochloride is a third-generation, non-sedating antihistamine that selectively antagonizes peripheral histamine H1 receptors. Histamine is a key mediator of allergic inflammation, released from mast cells and basophils during the allergic response. By competitively blocking the binding of histamine to H1 receptors on effector cells in the respiratory tract, skin, and gastrointestinal tract, fexofenadine prevents the development of typical allergic symptoms.

Unlike first-generation antihistamines and even some second-generation agents, fexofenadine is characterized by its inability to cross the blood-brain barrier. This is due to its zwitterionic nature and high polarity, which result from both positive and negative charge centers within the molecule at physiological pH. The lack of central nervous system penetration means fexofenadine does not cause the sedation, cognitive impairment, or psychomotor slowing associated with older antihistamines.

Fexofenadine is the active carboxylic acid metabolite of terfenadine, developed specifically to avoid the cardiotoxic side effects associated with its parent compound. It does not block cardiac potassium channels and has no known risk of QT interval prolongation. The drug acts rapidly after oral administration, with an onset of action within one hour and a duration of approximately 24 hours, allowing for once-daily dosing in most patients.

Therapeutic Uses & Indications

Therapeutic Uses

Fexofenadine hydrochloride is prescribed for:

  • Seasonal Allergic Rhinitis: Fexofenadine provides effective relief from sneezing, rhinorrhea, itchy nose and palate, and itchy, watery, red eyes associated with hay fever and seasonal pollens.
  • Perennial Allergic Rhinitis: For year-round allergies triggered by dust mites, pet dander, mold spores, and other indoor allergens, fexofenadine offers sustained symptom control.
  • Chronic Idiopathic Urticaria: Fexofenadine is highly effective in reducing the pruritus, number, and size of hives in patients with chronic urticaria of unknown cause.
  • Allergic Dermatitis and Eczema: As an adjunctive therapy, fexofenadine helps manage the itching associated with various allergic skin conditions.
  • Drug and Food Allergies (mild reactions): For mild allergic reactions to medications or foods, fexofenadine provides symptomatic relief.

Why Manufacturers Choose Fexofenadine Hydrochloride

Fexofenadine hydrochloride commands a significant share of the global antihistamine market, driven by its superior safety profile and non-sedating properties that make it suitable for daytime use. India has emerged as a leading manufacturing hub for fexofenadine API and finished formulations, with WHO-GMP certified facilities serving both domestic and export markets. The molecule is available in multiple strengths (120 mg and 180 mg), allowing manufacturers to offer both standard and double-strength options for PCD franchise distribution. Fexofenadine's strong brand recognition (Allegra) and established generic market make it a high-volume molecule with consistent demand from hospital formularies, retail pharmacies, and export channels. Its stability and well-characterized manufacturing process make it an ideal candidate for third party manufacturing in India.
