Mechanism of Action

Finasteride works as a competitive and specific inhibitor of the type II 5-alpha-reductase enzyme, a nuclear-bound steroid intracellular enzyme that converts testosterone into the more potent metabolite dihydrotestosterone (DHT). By blocking this conversion, finasteride significantly reduces serum DHT levels by approximately 70% and prostatic DHT levels by up to 90%. Since DHT is the primary androgen responsible for prostate growth and plays a key role in androgenetic alopecia, lowering its concentration produces therapeutic benefits for both conditions. Hair follicles contain type II 5-alpha-reductase, and in men with male pattern hair loss, the balding scalp shows elevated DHT levels. Finasteride decreases both scalp and serum DHT, inhibiting the process that leads to miniaturization of scalp hair follicles and potentially reversing the balding process. Unlike antiandrogens such as dutasteride which inhibits all three isoforms of 5-alpha-reductase, finasteride selectively targets the type II isoenzyme and has no affinity for the androgen receptor. The drug is rapidly absorbed with approximately 65% bioavailability, is extensively metabolized in the liver via CYP3A4, and has a serum half-life of 5 to 6 hours. Upon discontinuation of therapy, DHT levels return to normal within 14 days.
