Mechanism of Action

Fluoxetine works by selectively inhibiting the reuptake of serotonin (5-hydroxytryptamine or 5-HT) at the presynaptic neuronal membrane in the central nervous system. By blocking the serotonin transporter protein, fluoxetine increases the concentration of serotonin in the synaptic cleft, thereby enhancing serotonergic neurotransmission. This effect occurs through negative allosteric modulation of the serotonin receptor complex, which prevents the recycling of serotonin back into the presynaptic neuron. With chronic administration, the sustained elevation of synaptic serotonin leads to desensitisation and downregulation of postsynaptic 5-HT receptors, which is believed to be the mechanism underlying the antidepressant response. Unlike tricyclic antidepressants, fluoxetine has minimal affinity for adrenergic, histaminergic, and muscarinic receptors, which explains its more favourable side effect profile.
