Mechanism of Action
Fluvoxamine is a potent selective serotonin reuptake inhibitor (SSRI) that acts by blocking the serotonin transporter (SERT) on presynaptic neurons, preventing the reuptake of serotonin from the synaptic cleft. This increases extracellular serotonin concentrations and enhances serotonergic neurotransmission throughout the central nervous system. Fluvoxamine has approximately 100-fold selectivity for the serotonin transporter over the norepinephrine transporter and negligible affinity for the dopamine transporter, giving it a highly targeted mechanism of action. Uniquely among SSRIs, fluvoxamine also acts as a potent agonist at the sigma-1 receptor with a Ki of 36 nM, which may contribute to its antidepressant and anxiolytic effects while also providing benefits for cognitive symptoms associated with depression. It additionally increases concentrations of the neurosteroid allopregnanolone, further supporting its anxiolytic properties. Fluvoxamine is metabolized primarily by CYP2D6 and CYP1A2 and has a mean elimination half-life of approximately 15 hours, requiring once or twice daily dosing for effective treatment.
Therapeutic Uses & Indications
Therapeutic Uses
Fluvoxamine is prescribed for the treatment of:
- Obsessive-Compulsive Disorder (OCD): Fluvoxamine is FDA-approved for OCD in adults and children, effectively reducing obsessions and compulsions with sustained long-term efficacy.
- Major Depressive Disorder: Widely prescribed as a first-line antidepressant for moderate to severe depression, with efficacy comparable to other SSRIs.
- Social Anxiety Disorder: Approved for social anxiety disorder, helping patients manage fear and avoidance of social situations.
- Panic Disorder: Effective in reducing the frequency and severity of panic attacks with or without agoraphobia.
- Post-Traumatic Stress Disorder: Used off-label for PTSD symptoms, particularly hyperarousal and re-experiencing.
Why Manufacturers Choose Fluvoxamine
Fluvoxamine is one of the most widely prescribed SSRIs globally and is included in the WHO Model List of Essential Medicines, ensuring consistent demand across both domestic Indian and international markets. Its favorable side-effect profile compared to other SSRIs, particularly with respect to cardiovascular safety, makes it a preferred choice for patients with cardiac concerns. The availability of both immediate-release (50mg) and sustained-release (100mg) tablet formulations provides manufacturing flexibility and allows for brand differentiation. Indian pharmaceutical manufacturers serve a significant export market for fluvoxamine to Europe, Asia, and other regions, making it a commercially attractive molecule for WHO-GMP certified facilities. Its inclusion in pediatric OCD guidelines further expands the addressable market.
