Mechanism of Action
Furosemide is a potent loop diuretic that works by inhibiting the sodium-potassium-chloride (Na⁺-K⁺-2Cl⁻) cotransporter in the thick ascending limb of the loop of Henle in the kidney. This inhibition prevents the reabsorption of sodium, chloride, and potassium ions from the tubular fluid back into the bloodstream. The resulting increase in osmotic pressure within the renal tubule draws water along with these ions, producing a profound diuretic effect. Unlike thiazide diuretics that act on the distal convoluted tubule, furosemide's action on the loop of Henle gives it a much higher ceiling of efficacy, making it effective even in patients with moderate to severe renal impairment. The drug also causes increased excretion of magnesium and calcium, though to a lesser extent than sodium. Onset of action after oral administration is within 30 to 60 minutes, with peak effect at 1 to 2 hours and duration of 6 to 8 hours. The drug is highly bound to plasma proteins and is eliminated primarily by renal excretion as unchanged drug and as a glucuronide metabolite.
