Mechanism of Action

Goserelin is a synthetic decapeptide analog that closely mimics a natural hormone called gonadotropin-releasing hormone, which is produced by the hypothalamus in the brain. It belongs to a class of drugs known as GnRH agonists. When goserelin is first administered, it causes a temporary surge in the production of luteinizing hormone and follicle-stimulating hormone from the pituitary gland, leading to a short-term increase in sex hormone levels in both men and women. However, with continuous use over several weeks, the drug produces the opposite effect. The constant stimulation overwhelms the pituitary gland, causing it to become desensitized and reducing the number of GnRH receptors on its surface. This results in a sharp and sustained reduction in the secretion of luteinizing hormone and follicle-stimulating hormone. As a consequence, the testes in men dramatically reduce testosterone production, and the ovaries in women reduce estrogen and progesterone production to levels similar to those seen after surgical removal. This chemical castration effect is the therapeutic goal. Because many prostate cancers depend on testosterone and many breast cancers depend on estrogen to grow, depriving the tumor of these hormones effectively starves the cancer cells and slows or stops tumor progression. The drug is formulated as a biodegradable depot implant that releases goserelin slowly over one or three months, ensuring consistent suppression of hormone production throughout the treatment period.
