Mechanism of Action

Ifosfamide is a cytotoxic alkylating agent belonging to the oxazaphosphorine class, structurally related to cyclophosphamide. It acts as a prodrug that requires metabolic activation by hepatic cytochrome P450 enzymes, primarily CYP3A4 and CYP2B6, to generate active metabolites including isophosphoramide mustard and acrolein through hydroxylation at the ring carbon atom. Isophosphoramide mustard alkylates DNA by forming covalent bonds at guanine N-7 positions, creating interstrand and intrastrand DNA crosslinks that prevent DNA replication and transcription, ultimately triggering apoptosis in rapidly dividing cancer cells. The secondary metabolite acrolein contributes to cytotoxicity but is also responsible for urotoxicity, which is why ifosfamide is almost always administered with mesna as a uroprotective agent. Unlike some cell-cycle specific chemotherapy agents, ifosfamide's alkylating activity affects cells at all stages of the cell cycle.
