Mechanism of Action
Irbesartan works by selectively and competitively blocking the binding of angiotensin II to the angiotensin II type 1 receptor, which is the primary receptor responsible for mediating the classical pressor effects of the renin-angiotensin-aldosterone system. Angiotensin II is a potent vasoconstrictor that also stimulates aldosterone secretion, promotes sodium and water retention, and induces vascular smooth muscle cell proliferation and hypertrophy. By antagonising the AT1 receptor, Irbesartan prevents angiotensin II from exerting these effects, leading to relaxation of arterial and venous smooth muscle, dilation of peripheral blood vessels, and a subsequent reduction in systemic vascular resistance and arterial blood pressure. Unlike angiotensin-converting enzyme inhibitors, Irbesartan does not inhibit the degradation of bradykinin and therefore does not produce the dry cough that often limits ACE inhibitor therapy. The drug is highly selective for the AT1 receptor subtype over the AT2 receptor, leaving the AT2 receptor free to be stimulated by unopposed angiotensin II which may contribute to favourable vasodilatory and anti-proliferative effects. Irbesartan also demonstrates dose-dependent reductions in plasma aldosterone levels, contributing to its antihypertensive effect, and provides consistent 24-hour blood pressure control with once-daily dosing. The drug has additional renoprotective effects beyond blood pressure lowering, including reduction of intraglomerular pressure and proteinuria, making it particularly valuable in hypertensive patients with diabetic nephropathy.
