Lacosamide is a third-generation antiepileptic drug and a functionalized amino acid that exerts its anticonvulsant effect through a unique mechanism involving voltage-gated sodium channels. Unlike conventional sodium channel blocking antiepileptic drugs that affect fast inactivation, lacosamide selectively enhances the slow inactivation of voltage-gated sodium channels. This process occurs when the sodium channel transitions from an open conducting state to a non-conducting inactivated state over a period of seconds rather than milliseconds. By shifting the slow inactivation curve to more hyperpolarized potentials and increasing the proportion of channels in the slow inactivated state, lacosamide stabilizes hyperexcitable neuronal membranes and inhibits sustained repetitive neuronal firing without affecting normal low-frequency neuronal activity. This selective action on slow inactivation is particularly effective at suppressing the high-frequency firing patterns characteristic of epileptic discharges. Lacosamide also binds to collapsin response mediator protein-2, a phosphoprotein involved in neuronal differentiation and axonal outgrowth, although the contribution of this interaction to its anticonvulsant effects remains under investigation.
