Mechanism of Action
Lamivudine is a synthetic nucleoside analogue that exerts its antiviral activity against both human immunodeficiency virus type 1 and hepatitis B virus through intracellular conversion to its active triphosphate metabolite. Following oral administration, lamivudine enters cells and undergoes three sequential phosphorylation steps catalysed by cellular kinases to form lamivudine triphosphate. This active metabolite competes with the natural nucleoside deoxycytidine triphosphate for incorporation into growing viral DNA chains by the viral reverse transcriptase enzyme. Once incorporated, lamivudine triphosphate acts as a chain terminator because its modified oxathiolane ring structure prevents the formation of the 5-to-3 phosphodiester bond required for further nucleotide addition, thereby halting viral DNA synthesis. The drug exhibits stereoselective activity with the negative enantiomer being the active form. Lamivudine is a potent inhibitor of both HIV-1 reverse transcriptase and HBV polymerase, with the intracellular triphosphate having a half-life of approximately 17 to 19 hours in hepatocytes. Resistance develops through the M184V/I mutation in HIV-1 reverse transcriptase, which reduces susceptibility but also impairs viral replication capacity. Lamivudine is generally well tolerated and forms the backbone of many antiretroviral combination regimens due to its favourable safety profile and proven efficacy.
Therapeutic Uses & Indications
Therapeutic Uses
Lamivudine is prescribed for the treatment of:
- HIV-1 Infection: Lamivudine is indicated in combination with other antiretroviral agents for the treatment of HIV-1 infection in adults and paediatric patients. It is a core component of first-line and second-line antiretroviral therapy regimens worldwide.
- Chronic Hepatitis B Virus Infection: Lamivudine is indicated for adults with chronic HBV infection associated with evidence of viral replication and active liver inflammation, helping to suppress HBV DNA and normalise liver enzymes.
- Prevention of Mother-to-Child Transmission: Used as part of antiretroviral prophylaxis regimens to reduce the risk of vertical transmission of HIV from mother to infant during pregnancy and delivery.
- Post-Exposure Prophylaxis: Lamivudine is included in combination regimens for HIV post-exposure prophylaxis following occupational or non-occupational exposure to the virus.
Why Manufacturers Choose Lamivudine
Lamivudine is one of the most widely prescribed antiretroviral molecules globally and a cornerstone of WHO-recommended HIV treatment guidelines. Available in 30 mg, 100 mg, and 150 mg tablet strengths, it generates consistent high-volume demand from government antiretroviral programmes, institutional procurement, and retail prescription channels. Indian manufacturers are global leaders in antiretroviral production, with well-established API and formulation capabilities for NRTI molecules. The molecule's inclusion in multiple fixed-dose combinations further expands its market reach. With sustained demand from domestic PCD franchise networks and large-scale export contracts to Africa, Asia, and Latin America, lamivudine remains a volume-driven product with predictable manufacturing runs. This makes Lamivudine one of the most sought-after molecules for third party manufacturing in India, with consistent demand from PCD franchise operators and export-oriented companies.
