Mechanism of Action

Lansoprazole suppresses gastric acid secretion by irreversibly inhibiting the hydrogen-potassium adenosine triphosphatase (H⁺/K⁺ ATPase) enzyme system, commonly known as the proton pump, located on the secretory surface of gastric parietal cells. As a substituted benzimidazole prodrug, Lansoprazole is a weak base that accumulates in the acidic environment of the parietal cell canaliculus, where it undergoes protonation and conversion to its active sulphenamide form. This active species forms covalent disulphide bonds with cysteine residues on the alpha subunit of the H⁺/K⁺ ATPase enzyme, permanently inactivating the proton pump. Acid secretion resumes only after new proton pumps are synthesised and inserted into the membrane, typically over 24-48 hours. Lansoprazole is more potent than omeprazole on a milligram basis and has a faster onset of action due to its higher pKa, which enables more rapid accumulation in the parietal cell. The drug provides consistent 24-hour acid suppression with once-daily dosing and shows minimal food interaction, allowing flexible administration. Its anti-secretory effect also suppresses Helicobacter pylori growth indirectly, contributing to its role in eradication regimens.
