Mechanism of Action

Letrozole is a non-steroidal aromatase inhibitor that acts by competing with androgens for binding to the aromatase enzyme, thereby inhibiting the conversion of androstenedione and testosterone to estrone and estradiol in peripheral tissues. Aromatase is a cytochrome P450 hemoprotein-containing enzyme complex that catalyzes the rate-limiting step in estrogen biosynthesis, converting C19 androgens to C18 estrogens through a series of three hydroxylation reactions. Letrozole binds reversibly to the heme group of the aromatase enzyme with high specificity and affinity, with an inhibition constant in the low nanomolar range. It suppresses estrogen levels by 97 to 99 percent at therapeutic doses, a degree of suppression that is significantly more profound than that achieved with the first-generation aromatase inhibitor aminoglutethimide. Unlike steroidal aromatase inhibitors that form irreversible covalent bonds with the enzyme, letrozole binds non-covalently and does not require de novo enzyme synthesis for recovery of activity. Because letrozole does not affect the synthesis of corticosteroids, aldosterone, or thyroid hormone, it offers a favorable safety profile compared to earlier non-selective inhibitors. The reduction in circulating estrogen levels removes the primary growth stimulus for hormone-dependent breast cancer cells, leading to cell cycle arrest and apoptosis in estrogen receptor-positive tumors.

Therapeutic Uses & Indications

Therapeutic Uses

Letrozole is indicated for the treatment of:

  • Hormone Receptor-Positive Breast Cancer: First-line treatment for postmenopausal women with advanced or metastatic hormone receptor-positive breast cancer.
  • Adjuvant Therapy: Extended adjuvant therapy for early-stage hormone receptor-positive breast cancer in postmenopausal women after five years of tamoxifen therapy.
  • Neoadjuvant Therapy: Preoperative treatment to reduce tumor size in postmenopausal women with hormone receptor-positive breast cancer prior to surgical intervention.
  • Ovulation Induction: Off-label use for controlled ovarian hyperstimulation in women undergoing assisted reproductive technology, particularly in cases of anovulatory infertility.
  • Endometriosis: Used off-label as part of hormonal management for endometriosis-related pain and lesion suppression.

Why Manufacturers Choose Letrozole

Letrozole remains one of the most prescribed aromatase inhibitors globally, with consistent demand driven by the high prevalence of hormone receptor-positive breast cancer in postmenopausal women. Indian manufacturers benefit from well-established API supply chains and clearly defined IP, BP, and USP monographs that simplify regulatory submissions across domestic and international markets. The tablet formulation is straightforward to manufacture using conventional wet granulation or direct compression processes, making it accessible to facilities with standard oral solid dosage capabilities. Letrozole is an essential medicine included in the WHO Model List, ensuring steady institutional procurement demand. For Indian manufacturers targeting oncology product portfolios, letrozole tablets represent a foundational molecule that supports PCD franchise growth and export expansion into regulated markets across the Middle East, Africa, Southeast Asia, and Latin America.
