Mechanism of Action

Levothyroxine Sodium is a synthetic form of L-thyroxine (T4), the endogenous hormone produced by the thyroid gland. It acts as a replacement therapy in patients with thyroid hormone deficiency. Following oral administration, levothyroxine sodium is absorbed primarily in the jejunum and ileum and is extensively bound to plasma proteins, primarily thyroxine-binding globulin. The synthetic T4 is converted to the more biologically active triiodothyronine (T3) in peripheral tissues via deiodination by iodothyronine deiodinase enzymes, predominantly type 1 and type 2 deiodinases. T3 then enters target cell nuclei and binds to thyroid hormone receptors (TR-alpha and TR-beta), which are ligand-activated transcription factors. The T3-receptor complex heterodimerizes with retinoid X receptors and binds to thyroid hormone response elements in the promoter regions of target genes. This binding regulates the transcription of numerous genes involved in cellular metabolism, protein synthesis, oxygen consumption, thermogenesis, cardiac function, lipid metabolism, and neurological development.
