Mechanism of Action
Mefloquine Hydrochloride is a 4-quinolinemethanol antimalarial agent structurally related to quinine. It acts primarily by interfering with the parasite's ability to detoxify heme within the digestive vacuole of the malaria parasite. When the Plasmodium parasite invades red blood cells, it degrades haemoglobin as a source of amino acids, releasing free heme which is toxic to the parasite. The parasite normally polymerises this toxic heme into inert hemozoin crystals through a process called heme polymerase activity. Mefloquine binds to free heme and forms toxic complexes that inhibit hemozoin formation, causing oxidative damage to the parasite's membranes and organelles. The drug is active against the erythrocytic stages of all four human malaria species, including multidrug-resistant strains of Plasmodium falciparum. It does not kill gametocytes but does reduce their transmission potential. Mefloquine is well absorbed orally, has a very long elimination half-life of two to three weeks, and concentrates in red blood cells and tissues. Its prolonged half-life allows for weekly dosing in prophylaxis. The drug is effective against chloroquine-resistant falciparum malaria and is used both for treatment of acute uncomplicated malaria and for long-term prophylaxis in travellers to endemic regions.
