Mechanism of Action
Methylprednisolone is a synthetic glucocorticoid that exerts potent anti-inflammatory and immunosuppressive effects by binding to the intracellular glucocorticoid receptor alpha. Upon binding, the activated receptor-ligand complex translocates to the nucleus where it modulates gene transcription through two primary mechanisms: transactivation and transrepression. Transactivation involves binding to glucocorticoid response elements in the promoter regions of target genes, leading to increased transcription of anti-inflammatory proteins such as lipocortin-1, interleukin-10, and I-kappa-B-alpha. Transrepression involves interference with pro-inflammatory transcription factors, particularly nuclear factor-kappa-B and activator protein-1, resulting in reduced expression of pro-inflammatory cytokines including interleukin-1, interleukin-6, tumour necrosis factor-alpha, and interferon-gamma. Methylprednisolone also inhibits phospholipase A2 activity through induction of annexins, thereby reducing the release of arachidonic acid and subsequent production of prostaglandins, leukotrienes, and platelet-activating factor. The drug suppresses the migration of inflammatory cells to sites of inflammation, reduces capillary permeability, and stabilises lysosomal membranes. Its mineralocorticoid activity is significantly lower than that of hydrocortisone, making it suitable for high-dose anti-inflammatory therapy with reduced sodium-retaining effects.
Therapeutic Uses & Indications
Therapeutic Uses
Methylprednisolone is prescribed for a wide range of inflammatory and autoimmune conditions:
Rheumatic Disorders: Methylprednisolone is used in rheumatoid arthritis, juvenile idiopathic arthritis, ankylosing spondylitis, psoriatic arthritis, and acute gouty arthritis for rapid control of inflammation.
Allergic Conditions: It is indicated for severe seasonal or perennial allergic rhinitis, angioedema, urticaria, and drug hypersensitivity reactions unresponsive to conventional therapy.
Respiratory Diseases: Methylprednisolone is used in bronchial asthma exacerbations, chronic obstructive pulmonary disease exacerbations, aspiration pneumonitis, and sarcoidosis.
Dermatological Conditions: For pemphigus, severe erythema multiforme, exfoliative dermatitis, and severe psoriasis, methylprednisolone provides effective disease control.
Neurological Disorders: Methylprednisolone is indicated in multiple sclerosis relapses, cerebral oedema associated with brain tumours, and myasthenia gravis exacerbations.
Haematological Disorders: It is used in idiopathic thrombocytopenic purpura, autoimmune haemolytic anaemia, and acute lymphoblastic leukaemia as part of combination chemotherapy.
Nephrotic Syndrome: Methylprednisolone is employed in minimal change nephropathy and other glomerular diseases to induce remission.
Why Manufacturers Choose Methylprednisolone
Methylprednisolone is a high-volume corticosteroid molecule with broad therapeutic applications in both acute and chronic care settings. Indian pharmaceutical companies manufacture methylprednisolone in multiple strengths (4mg, 8mg, and 16mg tablets) to address diverse dosing requirements across different clinical conditions. The molecule is also available in injectable forms including methylprednisolone acetate and methylprednisolone sodium succinate for parenteral administration. Its inclusion in the WHO Essential Medicines List reinforces its clinical indispensability. For third party manufacturing partners, methylprednisolone offers consistent year-round demand from hospital supply chains, PCD franchise networks, and government health programmes. Indian WHO-GMP certified facilities produce methylprednisolone tablets for both domestic distribution and export to regulated and semi-regulated markets.
