Mechanism of Action

Metronidazole works by entering bacterial and protozoal cells and interacting with DNA. Once inside the cell, the nitro group of metronidazole is reduced by bacterial nitroreductases to form reactive intermediates including nitroso radicals and hydroxylamine derivatives. These reduced metabolites disrupt the helical structure of DNA and inhibit nucleic acid synthesis, leading to cell death. This mechanism is selectively toxic to anaerobic microorganisms because only anaerobes and certain microaerophilic organisms possess the necessary reducing systems to activate the drug. Aerobic bacteria lack these enzyme systems and are therefore resistant to metronidazole. The drug is bactericidal against obligate anaerobes including Bacteroides fragilis, Clostridium species, and Peptostreptococcus species. It is also highly effective against protozoan parasites such as Trichomonas vaginalis, Entamoeba histolytica, and Giardia intestinalis. Metronidazole is well absorbed orally with excellent tissue penetration, including across the blood-brain barrier, making it a first-line agent for anaerobic central nervous system infections.

Therapeutic Uses & Indications

Therapeutic Uses

Metronidazole is prescribed for the treatment of:

  • Anaerobic Bacterial Infections: It is the drug of choice for infections caused by obligate anaerobes including intra-abdominal infections, pelvic inflammatory disease, lung abscess, and brain abscess. It is often used in combination with other antibiotics for polymicrobial infections.
  • Bacterial Vaginosis: Metronidazole is the first-line treatment for bacterial vaginosis, available in both oral and topical formulations.
  • Protozoal Infections: Highly effective against Trichomonas vaginalis causing trichomoniasis, Entamoeba histolytica causing amebiasis, and Giardia intestinalis causing giardiasis.
  • Helicobacter pylori Eradication: Used as part of combination therapy for peptic ulcer disease caused by H. pylori infection.
  • Clostridioides difficile Infection: Oral metronidazole is used for mild to moderate C. difficile-associated diarrhea.
  • Acne Rosacea: Topical metronidazole is effective in reducing inflammatory lesions in rosacea.
  • Periodontal Infections: Used in dental infections and periodontitis, often in gel formulations for local application.

Why Manufacturers Choose Metronidazole

Metronidazole is one of the most widely manufactured antibiotics globally, featuring on the WHO Essential Medicines List. Its broad-spectrum activity against anaerobes and protozoa, coupled with excellent oral bioavailability and a well-established safety profile, makes it a staple product for any pharmaceutical manufacturer. The drug is available in multiple dosage forms including tablets, capsules, topical gels, creams, injections, and suspensions, offering manufacturers flexibility in production lines. Indian manufacturers benefit from high domestic demand and robust export markets across Africa, Southeast Asia, and the Middle East where parasitic and anaerobic infections remain prevalent. This makes Metronidazole one of the most sought-after molecules for third party manufacturing in India, with consistent demand from PCD franchise operators and export-oriented companies.
