Mechanism of Action

Montelukast Sodium is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene type 1 (CysLT1) receptor. Cysteinyl leukotrienes (LTC4, LTD4, LTE4) are potent inflammatory mediators derived from arachidonic acid metabolism through the 5-lipoxygenase pathway. These leukotrienes are released from various cells including eosinophils, mast cells, macrophages, and basophils, and they play a central role in the pathophysiology of asthma and allergic rhinitis.

By binding with high affinity and selectivity to the CysLT1 receptor, Montelukast blocks the bronchoconstrictor, pro-inflammatory, and mucus-secreting effects of leukotrienes in the airways. This results in reduced airway inflammation, decreased bronchial hyperresponsiveness, and improved pulmonary function. Unlike corticosteroids, Montelukast does not directly suppress the immune system but rather targets the specific leukotriene-mediated pathway of inflammation.

Montelukast is well-absorbed after oral administration and exhibits rapid onset of action. It is effective for maintenance therapy in persistent asthma and provides consistent symptom control throughout the day. Its once-daily dosing regimen and favorable safety profile make it a preferred add-on therapy for patients whose asthma is not adequately controlled on inhaled corticosteroids alone.

Therapeutic Uses & Indications

Therapeutic Uses

Montelukast Sodium is prescribed for the treatment of:

  • Bronchial Asthma: Used as maintenance therapy for the prophylaxis and chronic treatment of asthma in adults and pediatric patients. It prevents daytime and nighttime symptoms and reduces the frequency of asthma exacerbations.
  • Exercise-Induced Bronchoconstriction (EIB): Prevents bronchospasm triggered by physical exertion in patients aged 6 years and older.
  • Allergic Rhinitis: Provides relief from seasonal and perennial allergic rhinitis symptoms including sneezing, nasal congestion, rhinorrhea, and itching of the nose.
  • Urticaria (Hives): Used off-label for chronic idiopathic urticaria where antihistamines alone are insufficient.

Why Manufacturers Choose Montelukast Sodium

Montelukast Sodium is one of the highest-volume respiratory molecules in the Indian pharmaceutical market. With millions of asthma and allergy patients across India, the molecule enjoys massive prescription demand across all age groups. The availability of multiple strengths (4 mg, 5 mg, and 10 mg) allows manufacturers to serve pediatric, adolescent, and adult patient populations from a single production line.

The molecule has a well-established API supply chain with multiple domestic manufacturers, ensuring price stability and reliable sourcing. Its inclusion in PCD franchise portfolios is nearly universal, making it a must-stock molecule for franchise partners. Montelukast Sodium is also exported extensively to regulated markets across Asia, Africa, and Latin America. This makes Montelukast Sodium one of the most sought-after molecules for third party manufacturing in India, with consistent demand from PCD franchise operators and export-oriented companies.
