Mechanism of Action
Ornidazole is a synthetic nitroimidazole antibiotic with potent activity against anaerobic bacteria and protozoal parasites. It works through a reductive activation mechanism that selectively targets anaerobic organisms.
Ornidazole enters microbial cells by passive diffusion. Inside the cell, the nitro group at position 5 of the imidazole ring undergoes reduction by bacterial nitroreductase enzymes, a process that occurs only under low-oxygen conditions. This reduction produces nitroso radicals and other reactive intermediate compounds that are cytotoxic. These metabolites interact with microbial DNA, causing helical destabilization, strand breakage, and inhibition of nucleic acid synthesis. The result is rapid bactericidal activity against susceptible anaerobic organisms.
Ornidazole is more well-tolerated than Metronidazole, with fewer gastrointestinal side effects and a longer plasma half-life allowing once-daily or twice-daily dosing. This favorable tolerability profile makes it preferred for prolonged courses of therapy, such as in surgical prophylaxis or pelvic infections. Its antimicrobial spectrum covers Bacteroides fragilis, Clostridium species, Fusobacterium, Peptostreptococcus, and other clinically significant anaerobes, as well as protozoa such as Giardia lamblia, Entamoeba histolytica, and Trichomonas vaginalis.
Unlike Metronidazole, Ornidazole has a chloroethyl group that enhances its lipophilicity and tissue penetration, including effective penetration into cerebrospinal fluid, bone, and abscess cavities.
Therapeutic Uses & Indications
Therapeutic Uses
Ornidazole is prescribed for the treatment of anaerobic bacterial infections and protozoal infections:
- Anaerobic Bacterial Infections: Intra-abdominal infections (peritonitis, appendicitis), gynecological infections (endometritis, pelvic abscess), post-surgical wound infections, and severe odontogenic infections.
- Protozoal Infections: Amoebiasis (intestinal and hepatic), giardiasis, trichomoniasis, and balantidiasis.
- Surgical Prophylaxis: Pre-operative and post-operative prevention of anaerobic infections in colorectal, gynecological, and abdominal surgeries.
- Crohn's Disease: Used in management protocols after bowel resection to prevent recurrence of anaerobic overgrowth.
- Brain Abscess: As part of combination therapy for cerebral abscesses involving anaerobic organisms, given its excellent CSF penetration.
Why Manufacturers Choose Ornidazole
Ornidazole infusion is a staple product in hospital antibiotic formularies across India. Its superior tolerability compared to Metronidazole, longer half-life, and broader tissue distribution make it a preferred choice for clinicians treating serious anaerobic infections. The 100ml IV infusion format is a standardized item with year-round institutional demand. Indian manufacturers benefit from well-established API supply chains, cost-effective production processes, and strong export demand to Commonwealth and Middle Eastern markets where Ornidazole is widely prescribed.
This makes Ornidazole a consistently sought-after molecule for third party manufacturing in India, with steady procurement from hospitals, nursing homes, and PCD franchise operators.
