Mechanism of Action

Ornidazole is a synthetic nitroimidazole antibiotic with potent activity against anaerobic bacteria and protozoal parasites. It works through a reductive activation mechanism that selectively targets anaerobic organisms.

Ornidazole enters microbial cells by passive diffusion. Inside the cell, the nitro group at position 5 of the imidazole ring undergoes reduction by bacterial nitroreductase enzymes, a process that occurs only under low-oxygen conditions. This reduction produces nitroso radicals and other reactive intermediate compounds that are cytotoxic. These metabolites interact with microbial DNA, causing helical destabilization, strand breakage, and inhibition of nucleic acid synthesis. The result is rapid bactericidal activity against susceptible anaerobic organisms.

Ornidazole is more well-tolerated than Metronidazole, with fewer gastrointestinal side effects and a longer plasma half-life allowing once-daily or twice-daily dosing. This favorable tolerability profile makes it preferred for prolonged courses of therapy, such as in surgical prophylaxis or pelvic infections. Its antimicrobial spectrum covers Bacteroides fragilis, Clostridium species, Fusobacterium, Peptostreptococcus, and other clinically significant anaerobes, as well as protozoa such as Giardia lamblia, Entamoeba histolytica, and Trichomonas vaginalis.

Unlike Metronidazole, Ornidazole has a chloroethyl group that enhances its lipophilicity and tissue penetration, including effective penetration into cerebrospinal fluid, bone, and abscess cavities.
