Mechanism of Action
Pancreatin is a concentrated mixture of pancreatic enzymes obtained from porcine pancreas, containing standardized amounts of protease, amylase, and lipase. It functions as pancreatic enzyme replacement therapy (PERT) by replenishing deficient endogenous pancreatic enzymes. The lipase component hydrolyzes dietary triglycerides into fatty acids and monoglycerides, a critical step for fat absorption that cannot be bypassed by other digestive mechanisms. The protease component, comprising trypsin and chymotrypsin, cleaves dietary proteins into peptides and amino acids. The amylase component breaks down complex starches and glycogen into maltose, maltotriose, and limit dextrins. The enzyme activity is measured in USP units, with pancreatin typically standardized to contain not less than 25 times its weight of potato starch conversion capacity, 25 times its weight of casein digestion capacity, and defined lipase activity. The enzymes are active in the neutral to slightly alkaline pH of the duodenum, where they work optimally at pH 6.0 to 8.0. Without adequate pancreatic enzyme replacement, patients with EPI experience steatorrhea, malnutrition, and weight loss.
Therapeutic Uses & Indications
Therapeutic Uses
Pancreatin is prescribed for the treatment of:
- Exocrine pancreatic insufficiency (EPI): The primary indication, caused by chronic pancreatitis, cystic fibrosis, pancreatic cancer, pancreatic surgery, or Shwachman-Diamond syndrome.
- Steatorrhea: Helps manage fatty, loose stools associated with fat malabsorption, improving stool consistency and reducing stool frequency.
- Post-pancreatectomy digestion: Essential for patients who have undergone partial or total pancreatectomy and cannot produce sufficient endogenous enzymes.
Why Manufacturers Choose Pancreatin
Pancreatin is a high-volume digestive enzyme product with sustained demand across both pharmaceutical and nutraceutical markets. India is a major manufacturing hub for pancreatin, with several established API manufacturers producing the enzyme complex from porcine pancreas under WHO-GMP conditions. The tablet formulation at 212.5mg with standardized enzyme activity (protease 4500 FIP, amylase 6000 FIP, lipase 300 FIP units) is the most commonly manufactured form. Pancreatin third party manufacturing in India serves both the domestic market and export destinations across Asia, Africa, and the Middle East. The product is available under IP, BP, and USP grades, allowing manufacturers to target different regulatory markets. The growing prevalence of pancreatic disorders, increasing awareness of EPI, and expanding geriatric population contribute to consistent demand growth for pancreatin formulations.
