Mechanism of Action

Paroxetine is a selective serotonin reuptake inhibitor (SSRI) that exerts its antidepressant and anxiolytic effects primarily by blocking the serotonin reuptake transporter (SERT) located on presynaptic neuronal membranes. At higher doses exceeding 40 mg daily, paroxetine also demonstrates clinically significant inhibition of the norepinephrine transporter, contributing to its noradrenergic effects. It displays weak anticholinergic activity through minor affinity for muscarinic receptors, which may contribute to its more rapid onset of anxiolytic action compared to other SSRIs but also to side effects such as dry mouth and constipation. Paroxetine is a potent inhibitor of CYP2D6, leading to nonlinear pharmacokinetics where small dose increases can produce disproportionate rises in plasma concentration.
