Mechanism of Action
Perindopril is a non-sulfhydryl angiotensin-converting enzyme (ACE) inhibitor administered as a prodrug that undergoes rapid hepatic hydrolysis to its active metabolite, perindoprilat. Perindoprilat exerts its antihypertensive effect by competitively inhibiting ACE, a peptidyl dipeptidase enzyme responsible for converting the inactive decapeptide angiotensin I into the potent vasoconstrictor angiotensin II. By blocking this conversion, perindoprilat reduces circulating angiotensin II levels, leading to decreased peripheral vasoconstriction and lower systemic vascular resistance. Additionally, reduced angiotensin II levels diminish aldosterone secretion from the adrenal cortex, resulting in decreased sodium and water retention, lower blood volume, and a further reduction in blood pressure.
ACE is identical to kininase II, the enzyme that degrades bradykinin, a potent vasodilator peptide. Perindoprilat-mediated ACE inhibition therefore also increases bradykinin levels, contributing to its vasodilatory effects through activation of the kallikrein-kinin system and subsequent prostaglandin release. This bradykinin potentiation is thought to partially account for the therapeutic efficacy of ACE inhibitors as well as their characteristic side effect of dry cough. Perindopril produces dose-dependent ACE inhibition of 80 to 90 percent at 8 mg, persisting for 10 to 12 hours, with approximately 60 percent inhibition maintained at 24 hours. Unlike some other ACE inhibitors, perindopril has a prolonged terminal elimination half-life of 30 to 120 hours due to slow dissociation from tissue ACE binding sites, allowing once-daily dosing with consistent 24-hour blood pressure control. Perindopril also demonstrates beneficial effects on vascular structure and function beyond blood pressure reduction, including regression of left ventricular hypertrophy and improvement in arterial compliance.
