Mechanism of Action

Permethrin works by disrupting the neuronal membranes of ectoparasites, specifically scabies mites (Sarcoptes scabiei) and lice. It is a synthetic pyrethroid derivative modeled after the natural insecticide pyrethrin. Permethrin acts as a neurotoxin by binding directly to voltage-gated sodium channels in the nerve membranes of the parasites. Under normal conditions, these sodium channels open and close rapidly to conduct nerve signals. Permethrin prevents the inactivation gates of these channels from closing, delaying the repolarization of the neuronal membrane.

This action causes a prolonged influx of sodium ions, which results in continuous membrane depolarization, repetitive nerve firing, and synaptic hyperexcitation. The downstream consequence is rapid, progressive paralysis of the parasite's nervous and muscular systems, culminating in the death of the ectoparasites and their ova.

Unlike the parasites it targets, Permethrin exhibits extremely low toxicity in humans because mammalian cells rapidly metabolize it through ester hydrolysis and conjugate formation.
