Mechanism of Action

Phenytoin sodium is a hydantoin derivative anticonvulsant that stabilizes neuronal membranes and limits seizure propagation through voltage-dependent blockade of voltage-gated sodium channels. The primary mechanism involves binding to the inactivated state of the sodium channel during periods of sustained high-frequency neuronal firing, which characterises epileptic activity. By selectively prolonging the inactivation phase of sodium channels, phenytoin reduces the influx of sodium ions into neurons, thereby preventing the repetitive firing of action potentials that underlies seizure spread. The drug's action is most pronounced at the motor cortex, where it inhibits the spread of seizure activity from epileptic foci to adjacent cortical areas by reducing post-tetanic potentiation at synapses. Phenytoin also reduces the maximal activity of brainstem centers responsible for the tonic phase of generalized tonic-clonic seizures. Beyond its anticonvulsant properties, phenytoin exhibits class IB antiarrhythmic activity similar to lidocaine, prolonging the effective refractory period of cardiac tissue while improving atrioventricular conduction.
