Mechanism of Action

Pregabalin works by binding with high affinity to the alpha-2-delta subunit of presynaptic voltage-gated calcium channels in the central nervous system. Although structurally similar to the inhibitory neurotransmitter GABA, pregabalin does not bind directly to GABA-A or GABA-B receptors. Instead, its binding to the alpha-2-delta subunit reduces depolarization-induced calcium influx into nerve terminals, which in turn decreases the release of several excitatory neurotransmitters including glutamate, noradrenaline, serotonin, dopamine, and substance P. This modulation of neurotransmitter release is believed to underlie pregabalin's anticonvulsant, analgesic, and anxiolytic effects. The drug exhibits linear and predictable pharmacokinetics with high oral bioavailability, rapid absorption, and no significant hepatic metabolism. Pregabalin is not protein-bound and is excreted virtually unchanged by the kidneys, giving it a low potential for drug-drug interactions. Unlike many anticonvulsants, pregabalin does not induce or inhibit cytochrome P450 enzymes. Its therapeutic effects are observed across multiple pain pathways, making it effective for neuropathic pain conditions that are often refractory to conventional analgesics. The S-enantiomer of 3-isobutylgaba is the pharmacologically active form, with approximately ten times higher affinity for the alpha-2-delta site than the R-isomer.

Therapeutic Uses & Indications

Therapeutic Uses

Pregabalin is prescribed for the treatment of:

  • Neuropathic Pain: Pregabalin is FDA-approved for diabetic peripheral neuropathy and postherpetic neuralgia. It is also widely used off-label for other neuropathic pain conditions including trigeminal neuralgia, HIV neuropathy, and chemotherapy-induced peripheral neuropathy.
  • Fibromyalgia: Pregabalin was the first FDA-approved medication for fibromyalgia, providing significant pain reduction and improved quality of life in affected patients.
  • Epilepsy: As adjunctive therapy for partial-onset seizures in adults, pregabalin reduces seizure frequency when added to existing antiepileptic regimens.
  • Generalized Anxiety Disorder: Pregabalin is approved in many countries for GAD, offering rapid anxiolytic effects without the delayed onset typical of SSRIs.
  • Spinal Cord Injury Pain: Guidelines recommend pregabalin as a first-line treatment for central neuropathic pain following spinal cord injury.
  • Restless Legs Syndrome: Emerging evidence supports pregabalin's efficacy in managing RLS symptoms.

Why Manufacturers Choose Pregabalin

Pregabalin represents one of the highest-volume neuropathic pain molecules in the Indian pharmaceutical market. Its patent expiry has opened significant opportunities for generic manufacturers, with the Indian market seeing rapid growth in PCD franchise and third party manufacturing demand. The molecule offers multiple formulation options including immediate-release capsules, sustained-release tablets, and combination products with methylcobalamin and nortriptyline. IR strengths from 25mg to 300mg and SR strengths of 75mg and 150mg provide a comprehensive product portfolio that allows manufacturers to capture prescribing share across different patient segments. Pregabalin's established position in international treatment guidelines for neuropathic pain and fibromyalgia ensures sustained demand across both domestic and export markets. Indian manufacturers with WHO-GMP certified facilities and high-capacity tablet production lines are well-positioned to compete with competitive pricing.

This makes Pregabalin one of the most sought-after molecules for third party manufacturing in India, with consistent demand from PCD franchise operators and export-oriented companies.
