Mechanism of Action

Pyrimethamine and Sulfadoxine work synergistically to block two sequential steps in the folate synthesis pathway of the malaria parasite. This dual blockade is what makes the combination so effective against Plasmodium species.

Sulfadoxine, a long-acting sulfonamide, competitively inhibits dihydropteroate synthase by mimicking para-aminobenzoic acid. This prevents the parasite from synthesizing dihydrofolic acid, a precursor required for nucleic acid production. Pyrimethamine acts downstream by selectively inhibiting dihydrofolate reductase, preventing the conversion of dihydrofolate to tetrahydrofolate.
