Mechanism of Action

Rivaroxaban is a highly selective, direct factor Xa inhibitor that belongs to the direct oral anticoagulant class. It exerts its anticoagulant effect by directly binding to the active site of factor Xa, both free and clot-associated, thereby inhibiting the conversion of prothrombin to thrombin. This interruption of the coagulation cascade at a critical amplification point prevents fibrin clot formation without requiring antithrombin III as a cofactor. Unlike vitamin K antagonists such as warfarin, rivaroxaban does not require monitoring of coagulation parameters and has a predictable pharmacokinetic profile. The drug achieves rapid and reversible factor Xa inhibition with a Ki of 0.4 nM, demonstrating over 10000-fold selectivity for factor Xa over other serine proteases in the coagulation pathway. Rivaroxaban is rapidly absorbed with high oral bioavailability of 80 to 100% for the 10 mg dose, and peak plasma concentrations occur within two to four hours. Approximately two-thirds of the dose undergoes metabolic degradation primarily via CYP3A4 and CYP2J2, while one-third is eliminated unchanged via renal excretion. Its predictable dose-response relationship, fixed dosing regimen, and absence of routine coagulation monitoring make rivaroxaban a preferred anticoagulant for long-term use.
