Mechanism of Action
Rocuronium Bromide is a nondepolarizing neuromuscular blocking agent that produces skeletal muscle relaxation by competing with acetylcholine at the nicotinic cholinergic receptors of the motor end-plate. It binds with high affinity to these receptor sites, preventing acetylcholine from binding and initiating the muscle contraction cascade.
Normally, acetylcholine released from motor nerve terminals binds to nicotinic receptors on the muscle fiber membrane, triggering depolarization and muscle contraction. Rocuronium occupies these postsynaptic receptors without activating them, effectively blocking the transmission of nerve impulses to skeletal muscle. This competitive antagonism is dose-dependent, meaning higher doses produce a more complete and sustained neuromuscular blockade. The block can be reversed by acetylcholinesterase inhibitors such as neostigmine or sugammadex, which either increase local acetylcholine concentration or directly encapsulate rocuronium molecules.
