Mechanism of Action

Rosuvastatin is a selective and competitive inhibitor of 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase, the rate-limiting enzyme in the cholesterol biosynthesis pathway. By inhibiting this enzyme in the liver, rosuvastatin reduces the conversion of HMG-CoA to mevalonate, thereby decreasing hepatic cholesterol synthesis. This reduction in intracellular cholesterol triggers a compensatory upregulation of LDL receptors on hepatocyte cell surfaces, which enhances the clearance of circulating low-density lipoprotein cholesterol from the bloodstream. Rosuvastatin also inhibits the hepatic synthesis of very-low-density lipoproteins, further reducing the total pool of atherogenic particles. The drug has been designed with a polar methyl sulfonamide group that forms unique binding interactions with the HMG-CoA reductase enzyme, resulting in higher affinity and potency compared to many other statins.
