Mechanism of Action

Saxagliptin is a potent, selective, and reversible inhibitor of the enzyme dipeptidyl peptidase-4 (DPP-4), which belongs to the gliptin class of oral hypoglycemic agents. DPP-4 is a widely expressed enzyme that rapidly degrades incretin hormones such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP). These incretin hormones are released from intestinal L-cells and K-cells respectively in response to food intake. By inhibiting DPP-4, saxagliptin prolongs the half-life and increases the circulating levels of active GLP-1 and GIP. Elevated GLP-1 levels enhance glucose-stimulated insulin secretion from pancreatic beta-cells in a glucose-dependent manner — meaning insulin is only released when blood glucose is elevated, reducing the risk of hypoglycemia. GLP-1 also suppresses glucagon secretion from pancreatic alpha-cells, thereby reducing hepatic glucose production. The combined effect of increased insulin and decreased glucagon leads to improved glycemic control with a lower risk of hypoglycemia compared to sulfonylureas or insulin. Additionally, GLP-1 slows gastric emptying, which reduces postprandial glucose excursions. Saxagliptin exhibits approximately 400 to 950-fold selectivity for DPP-4 over related enzymes DPP-8 and DPP-9, which minimizes off-target toxicity concerns.
